People with mild, moderate or severe dementia may qualify. This overview reflects Medicare’s GUIDE participation criteria.
Quick eligibility check
You may qualify if:
- You have Medicare Part A and Part B
- You have diagnosed or suspected dementia
- You are not enrolled in Medicare Advantage, a Special Needs Plan or PACE
- You are not receiving the Medicare hospice benefit
- You live at home, in an apartment, assisted living or memory care
- You live within a Healthspan GUIDE service area
- You receive GUIDE services from only one participating program
