About Healthspan Partners

Guiding a better dementia experience.

Healthspan Partners is among a select group chosen by Medicare to provide GUIDE. Our mission is simple: improve quality of life for people living with dementia and support the caregivers beside them.

Good care starts by seeing the person, not just the diagnosis.

The GUIDE program delivers medically managed care coordination, personalized care planning, caregiver support and respite services, all at no cost to eligible Medicare beneficiaries.

We serve people with diagnosed or suspected dementia and the unpaid family members and friends who support them every day.
