GUIDE is a Medicare program designed to improve quality of life for people living with dementia and reduce strain on unpaid caregivers through coordinated care, education, 24/7 support and eligible respite services.
02Who may be eligible?+
People with diagnosed dementia who are enrolled in traditional Medicare may qualify. A quick eligibility check is the easiest place to begin.
03How much does Healthspan support cost?+
GUIDE services are available at no cost to eligible Medicare beneficiaries.
04What does a care navigator do?+
Your care navigator helps organize the plan, connect services, answer questions and keep the family and care team aligned.
05Is caregiver support included?+
Yes. Caregiver education, guidance and eligible respite services are core parts of the GUIDE experience.
